[B] Scope of Service:

(1) Vendor needs to provide homeless youth services that will focus on providing a community safe haven for youth in county, specifically homeless youth, to receive resource navigation and initial case management services to the government authority located in Salisbury, MD.
- Collaboration with community partners and established linkages to healthcare, shelters, workforce, education, and other resources to meet immediate and long-term needs.
- Facilitate school re-engagement, connection to adult basic education, or workforce development programs as appropriate to the individual youth.
- Provide linkages to emergency shelters, transitional housing, and housing stabilization programs.
- Assist with access to healthcare as needed.
- Peer input in program design and implementation.
- Utilization of promising practices, best practices and evidence-based practices demonstrating that the strategy has verifiable outcomes.
- Incorporation of intentional efforts to address racial and ethnic disparities.
(2) All questions must be submitted no later than May 25, 2022.
