(1) Vendor needs to provide chronic disease program evaluation services to the government authority located in Maine.
- Public health program evaluation services for the state center for disease control and prevention, diabetes and cardiovascular health programs
1. Mobilizing partners to support evidence-based prevention efforts
2. Gathering and disseminating data to drive healthy decision making
3. Empowering patients to self-manage chronic disease
4. Promoting evidence-based guidelines to strengthen health care systems
5. Linking health care systems with community resources;
6. Increasing access to screening for early detectable chronic diseases
7. Promoting evidence-based screening recommendations.
(2) All questions must be submitted no later than June 9, 2023.
(3) The contract period will be for two years.
