[*] Scope of Service:

(1) Vendor needs to provide housing case management services for demonstrated experience employing housing case management, which is essential to the success of the rental assistance program participants to the government authority located in California.
- Case management provides a blend of assessment and coordination services to help homeless individuals and families achieve housing stability and self-sufficiency.
- to provide case management to rapid re-housing and tenant-based rental assistance program participants.
- People experiencing homelessness or community groups providing another social service oriented program, qualifying education in social science, psychology, sociology, social and humans services, economics or other behavioral sciences may be substituted.
- Ability to apply theories of the motivational interviewing, harm reduction, and trauma informed approaches.
- Shall have excellent written and verbal communication and interpersonal skills and a proven ability to collaborate with community partners.
- Proficiency in Microsoft Office applications and HMIS.
- Shall be available to work evenings and weekends and provide services to clients on an emergency basis.
(2) All questions must be submitted no later than June 2, 2023.
(3) The contract term will be one years.
