[*] Scope of Service:
(1) Vendor needs to provide relocation assistance services to the government authority located in Austin, TX.
- Implementation of the Program consists of three phases.
- Phase 1: Program Initiation (Days 0 to 30)
A. Mobilize a team to perform Relocation Assistance services. Provider’s staff must be available to meet with Displaced Persons, also referred to as “Tenants,” in person and onsite at identified properties in the Subrecipients’ geographical area.
B. Deliverable: Provide a list of Provider staff, including their role and title, official job site locations, office hours and availability.
C. Participate in kick-off meeting with the agency and its vendors to discuss Program requirements.
D. Develop a system to track, manage, and report case management and associated Program tasks and statuses and for entry into the agency’s System of Record.
E. Open a bank account for the sole purpose of receiving and disbursing funds for Relocation Assistance payments. The selected Provider must maintain the account for the entirety of the contract period.
- Phase 2: Program Execution (Day 31 to Program Closeout)
A. Execute the approved outreach and engagement plan for current Tenants and Tenants that have already relocated from the acquired properties.
B. Deliverable: Prepare and submit to the agency for approval a written report identifying and describing the population of Displaced Persons from all acquired properties.
C. Perform all case management and customer service activities and tasks necessary to ensure Displaced Persons receive the full extent of URA protection and assistance.
D. Deliverable: Prepare and consolidate all necessary documentation for each Displaced Person into a URA File and submit it to the agency for review and approval. Update or revise documentation, as required.
- Phase 3: Program Closeout
A. Ensure the completion and documentation of all case files, deliverables, duties, and responsibilities in Phase 1 and 2.
B. Resolve all pending appeals, complaints, and customer service activities.
C. Ensure all URA Files have been uploaded into the agency system of record and transfer Program data to the agency system of record, if applicable.
(2) All the questions must be submitted no later than September 20, 2023.
